Título: Saúde do trabalhador e dor lombar : fatores associados e novas tecnologias analíticas.

Resumo: Introdução: As dores na região lombar podem ter relação com diversos fatores como inadequações posturais laborativas, cargas excessivas nesta região gerada pela obesidade abdominal e por desequilíbrios musculares, sendo que 80% da população sofre ou já sofreu com lombalgias. Objetivo geral: Caracterizar alterações posturais em trabalhadores rurais com dor lombar dos municípios da microrregião Sul do Conselho Regional de Desenvolvimento do Vale do Rio Pardo (COREDE/VRP), identificando a possível relação com variáveis antropométricas, bioquímicas e musculoesqueléticas. Artigo 1: Objetivo: Este estudo tem como objetivo analisar a presença de dor e a relação desta com parâmetros antropométricos, flexibilidade da região lombar e alterações posturais em trabalhadores rurais. Trata-se de um estudo transversal, descritivo e correlacional, em que foram sujeitos cinquenta e cinco trabalhadores rurais, dicotomizados conforme presença ou não de dor lombar, submetidos a avaliação antropométrica, da flexibilidade da região lombar e avaliação postural, analisando se há diferenças entre os grupos e se as variáveis tem relação com a dor lombar através da análise por componentes principais. Observou-se a presença de dor lombar em 67,2% dos trabalhadores, 29,7% eram obesos no grupo álgico versus 5,6% do outro grupo, a área de gordura visceral encontra-se elevada em 54,1% dos trabalhadores com dor lombar enquanto os que não apresentam, o percentual foi de 33,3%, a flexibilidade da região lombar não apresentou diferenças e 94,6% dos sujeitos com dor apresentaram alterações posturais. Através da análise por componentes principais as variáveis agruparam-se de formas diferentes, em que o grupo sem dor obteve relação no primeiro fator, principalmente com os parâmetros antropométricos referentes ao tecido muscular esquelético, e o grupo com dor com o tecido adiposo. Artigo 2: Objetivo: Associar a presença de dor lombar com a força dos músculos extensores lombares, ativação muscular da região lombar e parâmetros antropométricos em trabalhadores rurais. Este é um estudo transversal, descritivo e correlacional, em que participaram quarenta e oito trabalhadores rurais, separados em dois grupos: grupo sem dor lombar e grupo com dor lombar, os quais foram submetidos a avalição antropométrica, teste de força da musculatura extensora lombar e avaliação eletromiográfica do músculo longuíssimo do dorso (LD). Os resultados apontam diferença da força muscular lombar dos trabalhadores rurais em que o grupo sem dor lombar obteve maiores valores em relação ao outro grupo, verificou-se que a variável MME se relacionou positivamente com a força muscular dos extensores lombares e a ativação muscular do músculo LD. Já o IMC e os parâmetros antropométricos referentes ao tecido adiposo, exceto o %G, relacionaram-se negativamente com a ativação muscular do LD. O %G, bem como a intensidade da dor, além de terem relação negativa com a ativação muscular, também obtiveram relação negativa com a força muscular dos extensores lombares. Considerações finais: Os resultados obtidos apontam que, foi possível relacionar parâmetros antropométricos, e as alterações posturais com a dor lombar, pois há uma porcentagem maior de trabalhadores rurais com dor lombar apresentando obesidade e alterações posturais, e através da análise por componentes principais pode-se observar uma influência do tecido adiposo na presença de lombalgias, ainda, trabalhadores rurais com dor lombar apresentaram diminuição da força muscular extensora lombar e, em conjunto com as associações encontradas com os parâmetros antropométricos, sugere-se que o fortalecimento desta região pode influenciar a redução dos índices de dor lombar em trabalhadores rurais, bem como estratégias que promovam a saúde dos mesmos.
Resumo em outro idioma: Introduction: Lumbar pain may be related to several factors such as labor postural inadequacies, excessive loads in this region caused by abdominal obesity and muscular imbalances, with 80% of the population suffering or already suffered from low back pain. Objective: To characterize postural alterations in rural workers with low back pain in the municipalities of the Southern Region of the Regional Development Council of the Rio Pardo Valley (COREDE / VRP), identifying the possible relationship with anthropometric, biochemical and musculoskeletal variables. Article 1: Objective: This study aims to analyze the presence of pain and its relationship with anthropometric parameters, lumbar flexibility and postural changes in rural workers. It was a cross-sectional, descriptive and correlational study in which fifty-five rural workers, dichotomized according to the presence or absence of low back pain, submitted to anthropometric evaluation, lumbar flexibility and postural assessment were analyzed, analyzing whether there were differences between the groups and whether the variables are related to low back pain through principal component analysis. Lumbar pain was observed in 67.2% of the workers, 29.7% were obese in the pain group versus 5.6% in the other group, the area of visceral fat was elevated in 54.1% of the workers With lumbar pain while those without, the percentage was 33.3%, the flexibility of the lumbar region did not present differences and 94.6% of the subjects with pain presented postural alterations. Through the principal components analysis, the variables were grouped in different ways, in which the group without pain obtained a relation in the first factor, mainly with the anthropometric parameters referring to the skeletal muscle tissue, and the group with pain with the adipose tissue. Article 2: Objective: To associate the presence of low back pain with the strength of the lumbar extensor muscles, muscular activation of the lumbar region and anthropometric parameters in rural workers. This is a cross-sectional, descriptive and correlational study in which forty-eight rural workers participated, separated into two groups: a group without lumbar pain and a group with low back pain, which were submitted to anthropometric evaluation, lumbar extensor muscle strength test, and electromyographic evaluation of the very long muscle of the dorsum (LD). The results indicate a difference in the lumbar muscle strength of rural workers in which the group without lumbar pain obtained higher values in relation to the other group, it was verified that the MME variable was positively related to the muscular strength of the lumbar extensors and the muscular muscle activation LD. However, the BMI and the anthropometric parameters referring to adipose tissue, except the %fat, were negatively related to the muscular activation of LD. The %fat, as well as the intensity of the pain, in addition to having negative relation with the muscular activation, also obtained a negative relation with the muscular strength of the lumbar extensors. Final considerations: The results obtained indicate that it was possible to relate anthropometric parameters and postural alterations with low back pain, since there is a higher percentage of rural workers with low back pain presenting obesity and postural alterations, if there is an influence of adipose tissue in the presence of low back pain, still, rural workers with low back pain have decreased lumbar extensor muscle strength and, together with the associations found with the anthropometric parameters, it is suggested that the strengthening of this region can influence the reduction of low back pain rates in rural workers, as well as strategies that promote their health.
