Título: Saúde bucal de nefropatas : estudo transversal.

Resumo: Introdução: pacientes em terapia substitutiva necessitam de orientação em saúde bucal, a fim de evitar impactos negativos na doença renal e ainda colaborar para o sucesso de um possível transplante. Após o transplante, a imunossupressão pode facilitar que situações restritas à boca se tornem sistêmicas. Objetivo: analisar a saúde bucal de pacientes nefropatas em tratamento de hemodiálise e transplantados renais. Materiais e método: Trata-se de um estudo observacional transversal analítico, tendo como população de estudo pacientes em terapia substitutiva (hemodiálise) e transplantados renais do Serviço de Uronefrologia de Venâncio Aires - RS (SUNEVA) que foram submetidos a exame clínico intrabucal e questionário (em toda a amostra) e exame sorológico (em uma parte da amostra). Resultados: a amostra teve predomínio do sexo masculino (67,4%) e da raça branca (67,4%); a idade média dos indivíduos foi de 56,8 anos. Conforme ao tempo de hemodiálise, a maioria realizava a mais de três anos. Em relação às variáveis clínicas de toda a amostra, a média do CPO-D foi de 16,6; a de PC de 14,4; a de ISG de 0,71 e a de IPV de 39,1. Para análise dos exames sorológicos, foi utilizado o método de Quimioluminescência (CLIA) na detecção de anticorpos IgG e IgM reagentes ao herpes simples, os resultados dos exames de quatro pacientes transplantados e quatro em hemodiálise, demonstraram IgG positivo e IgM negativo para sete exames, resultando em imunidade antiga à doença, o qual já entraram em contato com o agente infeccioso, mas não demonstrando infecção recente ao herpes simples. O nível de significância utilizado como critério de aceitação ou rejeição nos testes estatísticos foi de 5% (p < 0,05). Conclusão: se tratando de pacientes favoráveis a problemas relacionados ao meio bucal, devido à xerostomia dos pacientes que fazem hemodiálise e aos imunossupressores que são utilizados pelos pacientes transplantados renais, e tendo em vista um perfil de higiene bucal precário, esses pacientes necessitam de melhor atenção odontológica e da conscientização de que as condições bucais podem alterar sua condição sistêmica.

After transplantation, immunosuppression may facilitate restricted mouth situations to become systemic. Objective: to analyze the oral health of nephropathic patients undergoing hemodialysis and renal transplantation. Materials and method: this is an observational cross-sectional analytical study. The study population consisted of patients undergoing substitution therapy (hemodialysis) and renal transplant patients from the Venâncio Aires Uronephology Service - RS (SUNEVA) who underwent intraoral clinical examination and questionnaire (in all sample) and serological examination (in one part of the sample). Results: the sample was predominantly male (67,4%) and white (67,4%); the average age of the individuals was 56,8 years. According to the time of hemodialysis, most performed more than three years. Regarding the clinical variables of the entire sample, the mean DMFT was 16,6; CP 14,4; ISG of 0,71 and IPV of 39,1. For the analysis of serological tests, the Chemiluminescence (CLIA) method was used to detect herpes simplex-reactive IgG and IgM antibodies. The results of four transplanted patients and four hemodialysis patients showed positive IgG and negative IgM for seven tests. resulting in old immunity to the disease, which have already come into contact with the infectious agent but not showing recent herpes simplex infection. The significance level used as the acceptance or rejection criterion in the statistical tests was 5% (p <0,05). Conclusion: for patients in favor of oral-related problems, due to the xerostomia of patients undergoing hemodialysis and the immunosuppressants that are used by renal transplant patients, and in view of a poor oral hygiene profile, these patients need better attention, dentistry and the awareness that oral conditions may alter its systemic condition.
