Título: Complexificação e aprendizagem na prática de semiologia médica.

Resumo: Pensar o processo de aprendizagem na prática de semiologia médica é o tema abordado nesta dissertação, tendo como objetivo compreender o processo de aprendizagem complexo do aluno do curso de medicina em atividades práticas numa instituição hospitalar de ensino na área da semiologia. Os pressupostos epistemológicos que sustentam este trabalho tem origem nas idéias de Humberto Maturana e Francisco Varela que demonstraram que a aprendizagem entre seres humanos acontece dentro das relações dialógicas, neste caso a relação médico paciente e Edgar Morin que enfatiza a teoria sobre a complexidade onde a aprendizagem ocorre num tecer de redes complexas de interações marcadas pela circularidade, não existindo uma causalidade linear, mas uma recursividade na relação médico-paciente. A partir da estrutura teórica constitui a questão central da pesquisa sendo: Como a prática de semiologia médica do aluno com o paciente no hospital no desenvolvimento das atividades num ambiente de perturbações construtivas, pode constituir-se num processo de aprendizagem complexo que integra todas as dimensões deste sujeito? O contexto empírico da pesquisa que originou este estudo foi uma unidade de internação adulta com aproximadamente 64 leitos, de um hospital geral localizado na cidade de Santa Cruz do Sul - RS. Foram selecionados para a participação dos processos investigativos um grupo de seis alunos do terceiro semestre do curso de graduação em medicina que iniciariam sua disciplina de Semiologia I, com prática na área hospitalar. A pesquisa foi de cunho qualitativo, sendo a metodologia aplicada através de entrevistas semi-estruturadas, e observação do que emergiu entre sujeito-sujeito na relação médico-paciente. A análise dos dados foi construída a partir de marcadores teóricos escolhidos pelo seu caráter operatório. Ficou evidenciado que os alunos vivenciaram uma aprendizagem prática significativa da semiologia médica, tendo como fio condutor a relação médico-paciente, promovendo mudanças cognitivas e afetivas a partir do acoplamento estrutural entre o aluno da semiologia com o paciente, ocorrendo assim a interação entre o ser/viver/conhecer/transformar-se.
Resumo em outro idioma: Thinking the learning process in a doctor-patient relationship is the main theme of this dissertation, having as the main goal to understand the complex learning process of the medical student during the practical activities in a adult unity of a learning hospital in the area of semiology. The epistemological assumptions that give support to this work come from the studies by Humberto Maturana and Francisco Varela who have showed that the learning among the human beings happens in the core of dialogical relationships, in this case the doctor-patient relationship. Edgar Morin, in a similar approach, emphasizes that in the complex theory the learning emerges in the interwoven of the complexes nets thought circular interactions, not existing therefore linear causality but recursively in the doctor-patient relationship. From the basic structure I constructed the central question of the research: How the doctor-patient relationship of the medical student with the patient in the hospital during the development of the practical activities in an environment of constructive disturbances can evolve toward a complex learning process that integrates all the dimensions of the subject? The empirical context of the research that originated this research was an unity of adults admission with around 64 beds of a general hospital at Santa Cruz do Sul - RS. The select students were 6 subjects from the third semester of the medical course Semiology I discipline to participate of the investigative process. The research was qualitative and the methodology was applied thought semi-structured interviews and observations that were emerged among the subjects in a process of doctor-patient relationship. The analysis of the data was done with the theoretical markers chosen due to theirs operatory feature. It was evident that the students lived a significative process of learning in medical semiology, having the doctor-patient relationship as guiding thread, promoting affective and cognitive changes from the structural coupling among the semiology student with the patient, occurring then the interaction being/living/ knowing/ transforming.
